Overview

Abstract:

Objectives: To describe clinical, subclinical characteristics and treatment results of patients with acute exacerbation of COPD with and without pneumonia at Thai Nguyen Central Hospital in 2020-2021. Subjects: 139 patients was diagnosed with ECOPD and treated at the Respiratory Department of Thai Nguyen Central Hospital. Methods: cross description. Results: The proportion of patients npECOPD accounted for 61.2%, pECOPD accounted for 38.8%. The disease was more in men (89.9%) than in women (10.1%). Mean age: 71.75±9.64 was mainly seen in patients aged 60 years and older, in which the age group over 70 years old accounted for the highest rate (61.2%). The proportion of patients regular treatment at home and having more than 2 exacerbations in the pECOPD higher than the npECOPD group. Fever in pECOPD (64.8%) was more common than in npECOPD group (8.2%). The results of chest X-ray showed that: pECOPD encountered mainly bronchial images, opacity, cloudiness; the npECOPD group only encountered bronchiectasis and alveolar lesions; lung increased brightness; low diaphragm. Mean WBC and mean CRP values ​​ in the pECOPD were higher than the npECOPD group. The mean hospital stay was: 8.68±3.66; 7-10 days (56.1%), less than 7 days ( 23.4%,) from 10 days or more (20.2%). Drug treatment in both groups of patients: The proportion of pECOPD patients using 2 antibiotics was higher than that of the npECOPD group (63.0% and 23.7% respectively). Good treatment results in the npECOPD (89.4%) were higher than in the pECOPD (72.2%). The treatment results were not good in the npCOPD (10.6%) than in the pECOPD (28.7%).
